The effect of physical work demands on cardiovascular disease in women. The Health and Career Path Survey, 2006 and 2010 waves, France.
Introduction: In France, cardiovascular disease (CVD) is the leading cause of death among women. The role of the workplace in the development of these conditions has been poorly documented. The objective of this study is to assess the effect of exposure to physical stressors at work on the incidence of CVD among women. Materials and Methods: Using data from the Health and Career Path (SIP) survey, which provides detailed information on working conditions, health status, and career trajectories, we utilized indicators related to night work, repetitive tasks, physical workload, and exposure to toxic substances, as reported in 2006. We then used a composite indicator reflecting exposures over the entire career. A binomial logit model was applied to 2,780 women and 2,633 men employed in 2006 to analyze the prevalence of CVD reported in 2010 and occurring after 2006. To account for confounding factors, individual characteristics in 2006 (age, occupation and socio-professional category, marital status, working hours, tobacco and alcohol use, body mass index), past characteristics (educational level and health), and employment status in 2010 were controlled for. Results: Among women, an association was found between exposure to harmful substances (OR=2.36) and at least two physical stressors (OR=1.98) and the reporting of CVD, as well as an association between exposure to toxic substances and hypertension (OR=1.84). The combined career indicator was significantly associated with a higher prevalence of CVD (OR=1.54) and HTN (OR=1.53). Except for demanding work (OR=1.35), we found no significant association with physical stressors among men. Conclusion: This finding calls for cardiovascular risk prevention strategies that specifically target women and involve stakeholders in the workplace.
