The Health of Socially Vulnerable Workers: A Pilot Project to Facilitate Access to Prevention and Care (Premtes), France, 2008–2010
A partnership between Occupational Health Services (SST) and Health Insurance Medical Examination Centers (CES) was piloted between 2008 and 2010, with the aim of facilitating access to preventive care and treatment for workers in socially vulnerable situations. The Epices score, an individual vulnerability score, enabled the identification of vulnerable workers during occupational health visits and the recommendation of a preventive health assessment by the CES. In total, 32 SSTs and 20 CESs participated in the pilot program; 192 occupational physicians administered the questionnaire to a random sample of 15,692 employees. More than 27% of workers were identified as vulnerable (4,272), and nearly a quarter of them received the preventive health assessment (1,015). Compared to non-vulnerable workers, workers in the pilot program had poorer health outcomes across most indicators: risk behaviors, lack of healthcare utilization, and perceived and measured health (statistically significant odds ratios ranging from 1.50 to 3.00). A satisfaction survey showed a positive evaluation of the pilot program. These results justify proposing an extension of the partnership to facilitate access to prevention services and help vulnerable workers remain employed. (R.A.)
