Residents admitted to nursing homes during the first quarter of 2013: conditions treated, treatments received, and hospitalizations in the following year.

Objectives: To characterize, based on healthcare utilization, the health status in 2013 of individuals in the year following admission to nursing homes for the elderly (Ehpad), as well as their medication regimens and the diagnoses associated with their hospitalizations. Methods: Individuals aged 65 and older admitted to an Ehpad in the first quarter of 2013, covered by the general health insurance scheme, and still alive one year after admission were selected from the Resid-ehpad database. Their reimbursed medical care during the 12 months preceding or following their admission was extracted from the Sniiram database, and 56 disease groups were identified using algorithms for the calendar years 2012 and 2013. Results: Among the 11,687 individuals studied (mean age 86 years, 76% women), neurodegenerative diseases (53%) and cardio-neuro-vascular diseases (51%) were the most common conditions. The use of psychotropic drugs (e3 prescriptions/year) increased after admission: antidepressants from 34% to 46%, anxiolytics from 32% to 42%, hypnotics/sedatives from 17% to 24%, antipsychotics from 10% to 21%, and systemic antibacterials (1 prescription/year) from 45% to 61%. The percentage of people hospitalized at least once dropped from 75% in the 12 months prior to admission to 40% in the 12 months following it. During hospitalizations following admission to a nursing home, the most frequent primary diagnoses involved diseases of the circulatory system and traumatic injuries (15% for each group), diseases of the respiratory system (11%), the digestive system (8%), and femoral neck fractures (7%). Discussion and conclusion: Medical-administrative data from the Resid-Ehpad/Sniiram linkage are useful for monitoring changes in the health status of nursing home residents, as estimated by healthcare utilization. Compared to recommendations, there is a high frequency and an increase in the use of certain classes of medications.
