SPIADI National Initiative. Observation of practices. Catheter insertion, catheter and line handling, and dressing changes for central catheters. Results of the OBSERVA4 survey conducted in 2025 and trends from 2023 to 2025
A total of 337 healthcare facilities participated in the 6th OBSERVA4 practice assessment campaign (+9% compared to 2024), and 7,645 observations were documented (+4% compared to 2024). For 45% of the national indicators assessing operator hygiene, patient hygiene, skin preparation, and preparation of the insertion site for catheter placement, line handling, and dressing changes for central catheters, exceeded 60% compliant observations, and 45% of these have shown significant improvement since 2023. More specifically, for central venous catheters, compliance rates exceed 60% for 15 of the 20 prevention measures studied. Significant improvement has been observed for 40% of the measures evaluated, including hand hygiene during dressing changes and proximal catheter manipulation. Regarding arterial catheters, 75% of the prevention measures evaluated have compliance rates above 60%, with significant improvement in hand hygiene during device insertion. For MIDline catheters with an integrated extension, 75% of the prevention measures evaluated had compliance rates > 60%, and improvement was significant in 20% of the prevention measures observed, including hand hygiene and the use of 2% alcohol-based chlorhexidine during device insertion. For needle insertion into a central venous catheter (CVC), 5 of the 6 prevention measures evaluated showed compliance rates > 60% (83%), with an improvement in hand hygiene. For short CVPs and MIDline catheters without an integrated extension, 10 of the 12 preventive measures evaluated showed compliance rates > 60% (83%), with significant improvement in 67% of them. These results help define priorities for action by 2026 and should encourage frontline professionals to use the SPIADI educational tools, which were developed to address identified training needs—particularly regarding the operator’s hand hygiene, the disinfection of connectors and valves prior to use.
