A Study of an Apparent Cluster of Crohn's Disease Cases in Indre-et-Loire, 1990–1999

Background: Following the diagnosis of four cases of Crohn’s disease (CD) between 1990 and 1997 among children living in Canton A of Indre-et-Loire, the local population raised hypotheses regarding a link between the cases and environmental factors. A study was conducted to investigate whether there was an excess of cases in this canton and to identify their potential determinants. Methods: New cases of CD (European definition) occurring between 1989 and 1998 in individuals residing in the Indre-et-Loire canton or the adjacent Vienne canton were identified through hospital departments, pediatricians, and gastroenterologists in both departments. The age- and sex-standardized incidence ratio (SIR) was calculated using data from the Northwest France Registry of Inflammatory Bowel Diseases. Statistical significance was assessed using a test designed for small sample sizes. A descriptive survey of the families of pediatric cases was conducted. Results: Four pediatric cases and four adult cases were identified, all from Canton A. In adults, the SIR was 0.45 (95% confidence interval [95% CI]: 0.12–1.15). Among children, the overall RIS was 8.51 (95% CI: 2.29–21.79) and 19.05 (95% CI: 5.12–48.70) in Canton A. The exploratory survey did not identify any exposure that would allow for a hypothesis regarding the origin of the cases. Conclusion: This study revealed a higher incidence among children in Zone A than that observed in France and enabled personalized contact with families affected by the disease, a social approach to the phenomenon, and the mitigation of rumors. It illustrates the systematic, step-by-step approach necessary for this type of investigation, which must also take into account the anxiety of the affected community. While the investigation of clusters must be integrated into the activities of public health agencies, in most cases, even if the results indicate an excess of cases, no conclusions can be drawn regarding the etiology.
