Evaluation of the system for monitoring and managing clusters of diseases with epidemic potential in long-term care facilities in Corsica. Winter season 2011–2012

In nursing homes for the elderly (Ehpad), a large number of cases and deaths caused by acute respiratory infections (ARI) and acute gastroenteritis (AGE) could be prevented by improving prevention and control measures. To this end, an awareness-raising campaign targeting Ehpads was carried out in Corsica for the first time during the winter of 2011–2012. This project aims to reduce mortality and morbidity through the early identification of clusters of cases, the implementation of reporting procedures to the Centers for the Coordination of the Fight Against Nosocomial Infections (CClin) and the Regional Health Agency (ARS), the implementation of appropriate control measures, and the investigation of outbreaks. This system was evaluated using a questionnaire. The response rate reached 60%. The survey shows that access to rapid diagnostic tests for influenza, although fairly widespread, was not available in all facilities. Furthermore, influenza vaccination coverage among residents was high, but too low for pneumococcal vaccination. Raising awareness of the importance of vaccination against Streptococcus pneumoniae to prevent severe infections is essential. Among staff, influenza vaccination coverage was completely insufficient, as was coverage against pertussis. Staff should be encouraged to get vaccinated, particularly by emphasizing the importance of limiting the spread of the virus within the facility, but also the benefits of protecting themselves against the disease and avoiding infecting their family members. Finally, it is essential to establish a culture of reporting ongoing clusters of cases in healthcare facilities to the Regional Health Agency (ARS) and the Regional Center for the Control of Nosocomial Infections (Arlin), as this enables these facilities to receive support or expert guidance.
