Invasive meningococcal infections in France in 2007. Regular surveillance reports - Infectious Diseases

In 2007, 721 cases of invasive meningococcal disease (IMD) were reported, which corresponds, after adjustment for underreporting, to an incidence rate of 1.3 cases per 105 inhabitants. Among these cases, 64% were under the age of 20. The highest incidence rates (per 105) were observed among children under 1 year of age (12.7), those aged 1–4 years (4.0), and those aged 15–19 years (3.1). Among the IIM cases for which the serogroup was known, 66% were serogroup B, 25% were serogroup C, 4% were serogroup W135, and 4% were serogroup Y. In 46% of all cases, the infection was meningitis alone; in 21% of cases, it was meningococcemia alone; and in 31% of cases, it was both meningitis and meningococcemia. Purpura fulminans was present in 27% of all cases, 26% of IIM B cases, and 33% of IIM C cases. The overall case-fatality rate was 12%, a level that has remained stable since 2003. It was higher in cases with purpura fulminans (28%, versus 6%, p<10⁻³). The case-fatality rate was 10% for IIM B and 17% for IIM C. The incidence of IIM in France in 2007 is comparable to that observed since 2004. Serogroup B remains predominant, and after increasing successively in 2003 and 2005, its incidence has remained stable. The incidence of IIM C, after peaking in 2002, gradually declined between 2002 and 2005 and has also been stable since then, despite a few localized epidemic and hyperendemic situations in 2006 and 2007 and the emergence of a new phenotype among strains belonging to the ST-11 clonal complex.
