Comorbidities, health status, and treatments among people with diabetes in France: data from the 2011 Sniiram survey.
Objectives: To estimate the prevalence of chronic conditions, health status, and treatments among people with diabetes in France. Methods: A mapping of patients and expenditures was developed by the National Health Insurance Fund for Salaried Workers using data from Sniiram. It identifies individuals receiving care for 56 groups of conditions (including diabetes), health states, and treatments (frequent, severe, or costly) among beneficiaries of the general health insurance system (including local mutual insurance sections) in 2011 (58 million people). Diabetes was identified using an algorithm based on the chronic condition "diabetes" or the presence of three reimbursements for oral antidiabetic medications or insulin in 2011 or 2010. The frequencies of the various conditions in the mapping among people with diabetes were calculated, then compared using age- and sex-standardized morbidity ratios (SMR) to those of the entire study population. Results: In 2011, among people with diabetes (4.7% of the population covered by the general health insurance scheme, mean age 65 years), 26% were treated for cardiovascular and cerebrovascular disease, a rate 1.5 times higher than that observed in the overall study population. Eleven percent were receiving care for a chronic respiratory disease (SMR=1.4), 5% for a psychiatric disorder (SMR=1.4), 3% for a liver or pancreatic disease (SMR=2.1), and 1% for end-stage chronic kidney disease (SMR=2.8). Discussion and Conclusion: People with diabetes are treated more frequently for certain chronic conditions than the general population, regardless of age. The presence of multiple comorbidities in people with diabetes complicates the medical management of diabetes. (R.A.)
