Tuberculosis in a nursing home: Use of an interferon-gamma release assay in contact tracing following a confirmed case

Introduction: According to current guidelines, the investigation of a tuberculosis case in a nursing home for the elderly (Ehpad) involves long-term follow-up of contacts through chest X-rays. Conducting this follow-up and repeating it over time is burdensome and remains imperfect among residents with reduced autonomy. The value of using an immunological test, the QuantiFERON®-TB Gold Plus (QFT-GIT), to diagnose latent tuberculosis infection (LTBI) in an investigation of a tuberculosis case in an Ehpad was evaluated. Method: A QFT-GIT and a tuberculin skin test (TST) were offered for LTI screening to all contacts of a case of bacilliferous pulmonary tuberculosis in a nursing home. Medical records of residents were reviewed for any acquired immunodeficiency. In cases of negative screening results, radiological follow-up was discontinued. Results: Thirty-five residents were included, with a mean age of 88 years and a male-to-female ratio of 0.18. None had comorbidities associated with immunosuppression; 5 patients had lymphopenia on blood counts. The positivity rate was 5% (2/35) for IDRs and 15% (5/35) for QFT-GITs. Three patients were treated for ITL; one patient refused treatment, and one patient died from neurological causes. This strategy led to the discontinuation of radiological follow-up for the 27 residents without lymphopenia who had negative immunological test results. Their clinical follow-up indicated no new cases. Conclusion: In the context of screening following a case of tuberculosis in a nursing home, performing a QFT-GIT for the detection of latent tuberculosis infection can be considered appropriate.
