Invasive meningococcal infections in France in 2011: key epidemiological characteristics

Introduction. For more than twenty-five years, the incidence of invasive meningococcal disease (IMD) in France has remained at approximately 1 case per 100,000 inhabitants. Recent data on cases occurring in 2011 are presented. Methods. Epidemiological surveillance of IIM relies on mandatory reporting to the French Institute for Public Health Surveillance and on the characterization of invasive strains by the National Reference Center for Meningococci. The completeness rate of mandatory reporting in metropolitan France has been estimated at over 90% since 2005. Results. In 2011, 574 IIM cases were reported, including 559 in mainland France, corresponding to an incidence rate, adjusted for underreporting, of 0.97 per 100,000 inhabitants. The median age of cases was 16 years. In mainland France, among the 530 IIM cases for which the serogroup was known, 72.8% were serogroup B, 15.5% were serogroup C, 2.6% were serogroup W135, 8.5% were serogroup Y, and 0.6% were rare serogroups. The case fatality rate was 10% in 2011 (57 deaths); it was 7% for serogroup B cases (28 deaths) and 17% for serogroup C cases (14 deaths). The incidence has remained virtually stable since 2005 for serogroups B and W135. Serogroup Y cases have been increasing since 2010. The last peak in IIM C occurred in 2003, and the decline in incidence observed between 2005 and 2010 continued only among children aged 1–14 in 2011. Conclusions. The incidence of IIM is moderate in France. IIM remains predominantly linked to meningococcal serogroup B. The decrease in the incidence of IIM C observed between 2010 and 2011 among children aged 1 to 14 years may reflect the impact of vaccination. However, the level of vaccination coverage achieved is insufficient to protect, through herd immunity, infants under 1 year of age who are not targeted by the recommendations of the vaccination schedule.
